Step 2: Assessment
During this stage of the admissions process, our team thoughtfully explores whether the Averte program is the right fit for the prospective resident. We take time to understand each person’s needs, strengths, and support requirements, and how those align with our program. This phase typically includes reviewing relevant records, connecting with family members or care providers when appropriate, and meeting with the individual either virtually or in person.
Step 3: Acceptance and Planning
Once a prospective resident is accepted, our team begins thoughtful transition planning to support a smooth and welcoming start at Averte. This stage focuses on developing an individualized plan of care, identifying supports, and coordinating move-in details. Our team works closely with the resident, their family, and care providers to clarify goals, establish routines, and ensure both the resident and staff are prepared for a successful beginning.
